[FAmSCo] Famsco survey ready

I got into lime survey service and created the survey. The service has a
option for printable survey. I used that option to create a sample of
the survey.

The survey is ready, I have only to activate it and start sending
request for people to take the survey.

I want a last minute review, to see if we are okey with the survey or if
there is anything else to incluse, something to exclude ...any comments?
