Fedora Appreciation Week : Contributor stories template

Hey CommOps,

We need to start collecting contributor stories from Fedora community for
Appreciation Week soon. Here is a rough draft of template for collecting
contributor stories:

Please add your thoughts on the template by next meeting 19-01-18.

Only after this, we can start working on a compilation of example stories
and writing a Community Blog teaser post about FAW.

I wondered if some people will have a hard time thinking of something to
write about. I added a few example scenarios to the "Description"
comment, but not sure if it's helpful. The rest looks good to me.
