Yes, add questions relating to their experience too.
Don't expect participants to understand "discriminatory behaviour" and "harassment" Break it down as per the scope of the survey.
"fairness in evaluation of work", "verbal abuse", "sexist remarks", ""etc attention must be paid to contexts : Online/Offline
Also add privacy related statements, disclaimers etc to the survey.
