Two suggestions:
- The receipt table format is confusing (when describing costs /

advise them to upload the receipts after opening the ticket, since I think they can only upload one file when opening a new ticket, but unlimited files when adding a comment.
Lastly, unrelated to the form, I might suggest defaulting issues to private for this repo, since it could contain private / sensitive information. You could use your own discretion if a ticket should be public or private later.
