First off, I was able to buy a SIM at the airport for 5zł, but, as Rafał pointed out, it was worthless. To actually use the SIM, I needed to activate it at a proper shop with real identification (i.e. my passport).
I ended up going to a Salon Play in a Kaufland at https://www.google.co m.lb/maps/search/salon+play/@50.0913723,19.8570229,12z/data=!3m1!4b1 wi th my passport and got a Play Internet na Kartę 9GB for 19zł ($4.83) plan. It doesn't seem to support LTE, but I get H and H+ just fine, and that's all I really need.
They also told me that there's some promotion that will give me 100GB for free for 30 days, if I type in a USSD code (*111*300*1#) tomorrow. We'll see if that actually works.
I did try T-Mobile first, but they told me I'd have to go to a store in the old city, while Play was willing to take my passport information and give me a SIM right there.

I would also recommend Play. Just checked and the offer you got is called "Play Internet na kartę".
As of 25th of July in Poland all the sim cards requires registration so it's better off getting this type of card in the proper selling point, here is the list from play (type Kraków to get list of points in that city):

LTE works with Play. I'm using it for 2 years. But if you have phone from outside EU market, LTE frequencies can be different and your phone could not support polish frequencies.

I'm not sure at which Play store you were, your map just displays all Play stores in the town, but Play employees had no reason to help you with T-mobile SIM, even they might have reasons not to help you. It looks like now you have a not-yet-registered T-mobile SIM you can give/sell to anybody. There are more than one T-mobile store unless you really were near the old city.
Though, I'm glad to learn you managed to run your Play card. I have no preference of a mobile provider.

OK, got it, I know that place. This is a small shopping center and I'm surprised to learn that Play is there, also not surprised that there is no other brand due to not enough space.

Strange, I've just checked and they had all brands but maybe it was different when you were. Whatever they had, as you said, the SIMs are inactive and cannot be activated immediately.

That's even more strange, the only explanation I can think of is that they did not yet know how to implement the new law or they were overwhelmed by the number of clients willing to buy a prepaid SIM.
