"Not approved" can be understood as "not yet decided". You might
get
requests to please evaluate and approve this or that not-approved
license.
Yep, same concern here.
Other than that, I also share Neals point, although this is the bigger
concern to me.

I think "disapproved" would be clearer. Or "forbidden",
"prohibited",
"rejected" or "disallowed".
