A little more research showed that at some stage totem-xine had been installed (I didn't do it, or at least not intentionally) and that totem was trying to use the xine backend.
I removed this package and all is good now.
I still can't view the menu's on DVD. I thought totem shoudl now do this. Should I file a bug report?

No, it doesn't do menus. It just gives you all the tracks listed in the playlist.
