What's the problem? Right click any menu item and select 'Add to Desktop'; and no, you don't have to add them to a folder view...am I missing something?
Are you talking about KDE 4 or KDE 3.5.x? AFAIR KDE 4 menu items can only be added to favorites.
No you can add them to desktop. But you need to unlock widgets.
