I've used FreeRADIUS in the past, its a very powerful and flexible radius server. We had it set up to do accounting and
authentication
against MySQL and Microsoft's SQL server (as well as some proxied requests to other radius servers). Also if you're looking for help check out the FreeRADIUS list serves. http://www.freeradius.org/list/users.html

Thanks Mike, where did you find the docs for Freeradius??
Much is included with the source (/usr/share/doc/freeradius-1.0.1/) but for the most part I just followed the comments in the config files found in /etc/raddb (if you install via rpm) Also there is a RADIUS O'Reilly book. I seem to remember it being very helpful.
