Fedora 17+ supports a separate /usr just fine. It's the separate /usr/lib that's screwing it up. (I suspect the upgrade process isn't mounting /usr/lib.) Since this is not a common configuration, you'll need to do a little extra work to accomplish an upgrade.
You need to instruct dracut to mount the necessary partitions during early boot. To do so, create a file /etc/fstab.sys and copy the lines for /, /usr, and /usr/lib from /etc/fstab to it. Then, create a file /etc/dracut.conf.d/02-fstab-sys.conf with this line:
add_dracutmodules += " fstab-sys "
(Note that spaces inside the quotation marks; they are necessary.)
