The license is here: http://creativecommons.org/licenses/by-sa/3.0/legalcode

Some relevant sections are 3b, which says that you need to take reasonable
steps to clearly identify that you have modified the original work, and 4c,
which covers how to handle attribution. (Which is "in any reasonable
manner", with some stipulations.)
