[Insight] insight02.dev puppet errors fixed?

After a great deal of hammering my head against the wall, I think I've
eliminated all the remaining puppet problems with insight02.dev.
However, I haven't turned on the AuthFAS plugin at this time, since I
can't be sure it won't break something else.  I'll look at that again
as soon as I have some more spare cycles.
