Normally (and nominally) it is created by
/sbin/install-info
Whether a package creates it depends upon its internals being used for
installing *.infos.
In some cases "make install" creates it, sometimes "make install-info"
creates it, sometimes you need to create it manually.
As far as rpms are concerned, the infamous PreReq: /sbin/install-info
and related %post/%preun etc. sections are addressing this issue.
