I think this file is in the linux-firmware package, which may not have
been installed. You might need to boot into rescue mode, install that
package, and regenerate the initrd.

Then again, the qlogicpti driver failing to load may have nothing to do
with your failure. I don't know what's hanging off that SCSI bus.
