A couple of times I saw something like this happen during FC3 test (or
maybe it was FC2 test). It turned out that my root partition, formatted
as XFS, was somehow getting corrupted.

Once I started using ext2 or ext3 instead of XFS for / (which contained
/boot), the problem stopped happening for me.

I still don't completely trust XFS in 2.6.10(-ac). But, 2.6.11 will have
lots of XFS fixes, so maybe XFS will really become usable in that
kernel...
