Always room for improvement, or at least for bouncing around ideas.
Please note the problems you've observed ... and if you have suggestions
for fixing ... don't hold back on those either.

My intent with the retrospective pages is so we document the problems as
they happen.  Even if we don't have brilliant ideas for resolving the
issue, it's helpful to see how many times the same issue surfaces, and
what has been attempted in the past.
