/ must be on a partition or LV that will be formatted. Reusing an existing / is not allowed.

When extents are available, they are allocated sequentially as you
create LVs.  There's no guarantee (since the extents could be fragmented
if you have created and removed LVs), but on an empty VG, you'll get
them that way.
