if you still have the installation file look at the readme.txt or
readme.html. It explains how to uninstall everything.

I've duplicated it here in case you have lost it:

Unistall instructions
---------------------

To remove the Plug-in Player for Linux:

For root users:
- Quit the browser.
- Navigate to the browser's plug-in directory (i.e.
/usr/local/mozilla/plugins).
(more likely /usr/lib/mozilla or /usr/lib/mozilla-1.4.1 on FC1)
- Remove libflashplayer.so and flashplayer.xpt.

For non-root users:
- Quit the browser.
- Navigate to your home directory and locate the folder named .mozilla.
- From that folder, open the folder named plugin and remove
libflashplayer.so, flashplayer.xpt and x-shockwave-flash.desktop.

To remove the Standalone Player for Linux:

For root users:
- Navigate to /usr/bin.
- Delete gflashplayer.

For non-root users:
- Navigate to your home directory and locate the folder named bin.
- Delete gflashplayer.

Finally, if all else fails, just run "updatedb" as root user than use
"locate gflashplayer" to find the file and then delete it. Repeat for
the plugin files.

HTH,
John
