... or there's a cable problem, or there's a problem with the IDE
adapter. I've got a motherboard with a dodgy IDE adapter that's only fit
for CD-ROMs that throws that problem occasionally.

The adapter is a HighPoint device on the PCI bus: the on-board IDE ports
work fine, so I haven't replaced it.
