Or perhaps some sort of automated process could do the bookkeeping of the
installation, behind the scene. 

For instance, store the detected hardware in some file, then maintain a
check list/log file for each step of the installation. If the installation
fails at some point, or if it encounters problems, these would be recorded
in the log file and the user could then send the logs to the developers. 
No forms to fill.
