After entering the contact information in the
Fedora Free Media Request Form

https://fedoraproject.org/freemedia/FreeMedia-form.html ,

 I tried submitting using the "Take me on to the next page"  button .

It gives the following error massage.
 Forbidden

TICKET_CREATE privileges are required to perform this operation.
