I'm assuming you are using EL 6.3 with the EPEL6 version of 389-ds-base?
I wonder if this bug is https://fedorahosted.org/389/ticket/410 or 
https://fedorahosted.org/389/ticket/405

I haven't updated the EPEL6 389-ds-base for a while - will do so now - 
is there any chance you could try to reproduce on a "newer" platform?
