USB modem driver ID additions to F15 and F14

Hi,

I'd like to request the following upstream patches be applied to F15
(and if possible F14) kernels:

(SHAs from linux-2.6.git)
cdacb598fe7ab85de80908c818dd7d66a2971117  option: add Zoom 4597 modem USB IDs
15badbcc8eede58b0d7e53a3acde1c90a7b6e40e  option: add Alcatel X200 to sendsetup blacklist
5c3e4076ee8253c1e3688d10653ddee47a03b0db  option: add Prolink PH300 modem IDs
7e8e62e4a5d26e4cb45f25dddd093837d75616c2  USB: option Add blacklist for ZTE K3765-Z (19d2:2002)

Simple ID additions and blacklist updates; should be a pretty straight
cherry-pick.

Looks ok to me. Feel free to commit.
