It seems that sometimes on reboot, pulse gets confused and thinks there already is a copy of the daemon running, and so doesn't start one.
As I've added to one of the tickets about that, here it was reproducible. At boot-time, the /tmp/pulse-$USER/pid file still existed and contained the pid number from before the shutdown, but a different process now got that pid.
There are several older and newer duplicates of that ticket. They have been closed in a chaotic way. One still open is: http://bugzilla.redhat.com/405351
